How To Qualify For Assistance

Residence

Family must permanently reside in the
State of Michigan.

Dependency

There must be a legally dependent minor (26 years of age or younger) in the patient’s home.

Diagnosis

Patient must be receiving active treatment for a cancer diagnosis*.

Need

Family must be able to show current or projected loss of income, or an increase in expenses, due to a cancer diagnosis.

* This may include chemotherapy, radiation, bone marrow transplant or surgery to remove the cancer. We do not consider reconstruction or hormonal therapy to be active treatment.

If you meet our program requirements, please reach out to a Social Worker or Nurse Navigator at the hospital at which you are receiving treatment. He/she will assist you in applying for financial assistance from our foundation.
